Subject: Change of health benefit coverages
Please be advised that the Department of Education represented by SACM has made some changes to your health benefits for 2017. The benefit administrator will continue to be UnitedHealthcare (UHC). The changes listed below go into effect January 1, 2017:
1. Physiotherapy benefits are being reduced from 60 visits per year to 12 visits per year to match what most US Colleges and University insurance plans offer.
2. Intrauterine insemination (IUI) is being reduced from 6 cycles per lifetime to 3 cycles per lifetime. Members will be able to move onto Advanced Reproductive Technology such as IVF after the 3 cycles of IUI are exhausted.
3. Applied Behavioral Analysis will be excluded from the policy unless a written exception is approved by SACM to UHC.
